Where are you located?

My office is 4421 Mormon Coulee Road, La Crosse,  WI 54601. Coulee Physical Therapy is the in Mormon Business Park building, the corner of Mormon Coulee and  Broadview Roads, across from Wendy’s on the south side of La Crosse.  Please come to the upper level parking lot, facing Mormon Coulee Road (not the lower level lot by Johnson Chiropractic). From there, park nearest the building and look for Coulee Physical Therapy signs to direct you to the correct entrance.

Do I need a referral from a physician?

No.  In the state of Wisconsin physical therapists have what is called Direct Access.  This means they can treat without a referral.

Can I request notes and can you communicate with my doctor should there be a need?

Yes absolutely.  I record detailed notes of my assessment and treatment performed each visit.  If there is a need to relay information to another provider I would be happy to assist.

Do you treat vestibular dysfunctions, including BPPV?

Yes, I have an advanced vestibular certification and many years of experience, and the necessary equipment and software to perform complete and thorough evaluations and treatment. Please note that vestibular cases are priced higher due to the need for advanced equipment and training.

Can I use my FSA or HSA to pay for treatment?

Absolutely!  I accept checks or debit/credit cards from your FSA/HSA.  You will be provided with the invoice and “Superbill”  with the required information that qualifies for reimburse from this type of account.  Thus far Coulee Physical Therapy hasn’t had any problem with qualifying you for my services, but this is not a guarantee and you are ultimately responsible for payment.

Can you help with a recent or acute injury?

Yes.  In some cases if you contact me directly I can evaluate your situation within 24 hours.  The techniques I use are apprpriate to help reduce initial inflammation and injury, and thereby shorten your recovery!

I have ongoing chronic pain.  Can you help?

Yes.  The techniques I use are aimed at whole body recovery to effect even the deepest layers of dysfunction.  If the condition is something you must manage over time, meaning it won’t ever go away, I may recommend more regular sessions, spread apart several weeks.  This not only is in your best interest for pain control, but also financially sensible. By staying ahead of your condition so it doesn’t get to the “boiling point,” you can avoid more severe acute flare ups which require more treatment.

Does it make sense to use the “Cash Based” Physical Therapy concept for my particular  needs?

There are several reasons people may choose to forego their insurance and use the cash concept.

  • They prefer staying with the same experienced provider for consistency of care.
  • They have a very high deductible and pay inflated cost for services otherwise, when they do not expect to meet their annual deductible.
  • They are accustomed to getting regular massages for health benefits, but feel they could use the services of a physical therapist to offer just a bit more skills, especially if they have an ongoing condition they need to manage.  They are willing to pay just a little bit more for increased level and selection of techniques.
  • The person has no insurance plan and this is the most cost-effective way when paying out of pocket for therapy services
